N modern houses, the combination of insulation and poor ventilation leads to high levels of indoor humidity, mite allergens, and volatile organic compounds. A study from Sweden examined the effects of mechanical supply and exhaust ventilation systems on humidity, allergen, and air pollutant levels in modern houses. The researchers measured the ventilation rate, indoor temperature, air humidity, mattress mite allergen concentrations, and total indoor volatile organic compounds in 59 modern houses in the Stockholm area. A mechanical supply and exhaust ventilation system was installed after construction in 22 of the houses. Only 5 of the 22 houses with mechanical ventilation systems had an air exchange rate per hour ACH ; of less than 0.5, compared with 24 of the 29 houses with natural ventilation. Absolute indoor humidity, mite allergen levels, and volatile organic compound levels were all significantly lower in the mechanically ventilated houses. For modern houses in cold climates, mechanical supply and exhaust ventilation systems increases the chances of reaching an ACH of 5 or greater. This level of air exchange rate lowers indoor humidity, making it more difficult for mites to survive, and reduces levels of indoor air pollutants in winter. This approach is most likely to succeed in areas with cold, dry winter climates, as dry air drawn in from outdoors will replace the humid air indoors, thus reducing indoor humidity levels. COMMENT: This Swedish study found that increasing mechanical ventilation in modern, highly insulated homes resulted in reduction of air humidity and a significant decrease in house dust-mite allergen levels. Although the study demonstrates the potential benefit of changes in building ventilation in cold temperate climates, these results may not necessarily apply to similar efforts in homes located in other climates. Expression of the AR, HPN, PSMA, and PSA genes in CTCs was surveyed serially over an 8- to 26-week period in three patients Fig. 4 ; . Two of the three patients had received chemotherapy. The results shown in Fig 4 demonstrate that the CTC counts reflect disease progression more accurately than semiquantitative PSA and PSMA RT-PCR results, which showed uniformly high expression throughout the monitoring period. This may be limited by the semiquantitative RT-PCR assay used in this study, however, because a relative gene expression score of 4 for PSA and PSMA can not be differentiated from saturation in this detection system. Our data imply that previous PSA RT-PCR studies that attempted to predict survival may have been hampered by an inability to differentiate between high and low CTC counts. The results further support research into AR as a therapeutic target because the amount of AR expression was sustained and found to be minimally affected by chemotherapy. In conclusion, we believe that gene expression profiling of CTC mRNA is feasible. Our data further validate that the immunomagnetically enriched CTCs enumerated by flow cytometry or fluorescence microscopy originated from the CaP tissue. The prostate CTCs exhibited heterogeneous expression of genes such as PSA, PSMA, and HPN. In the HRPC cases studied, AR expression appeared to be consistently increased, suggesting AR as a viable therapeutic target. The ability to quantify, phenotype, morphologically examine, and now molecularly profile CTCs could lead to improved characterization of HRPC and ultimately to development of more effective, personalized novel therapeutic strategies.
