Differential Gene Expression Patterns in Genital Fibroblasts of Normal Males and 46, XY Females with Androgen Resistance. The effector mechanisms downstream of AR-signaling that contribute to the normal or the abnormal male phenotype are poorly understood. Methods: Genital fibroblasts from 13 female patients with complete or partial AIS and defined defects of the AR and 9 normal males were cultured in the presence or the absence of dihydrotestosterone DHT ; . High-throughput mRNA-expression analyses were performed with 44, 000spot cDNA-microarrays. Genes with statistically significant differences in expression were identified using Single Value Decomposition and the Significance Analysis of Microarrays procedure. Results: We detected highly conserved transcriptional programs that readily differentiated gonadal fibroblasts from genital skin fibroblasts independent of the AR-status. A subset of approximately 400 genes also was identified that significantly discriminated male genital skin fibroblasts from 46, XY female genital skin fibroblasts except in 2 female patients with considerable expression of the wild type AR in their cultured labial cells due to somatic mosaicism. Gene expression patterns were not affected by DHT. Conclusion: The ontogenetic pathway of the genital fibroblasts is reflected in the pattern of gene expression suggesting spatial programming of mesenchymal cells in the morphogenesis of genital structures. In addition, androgen signaling during prenatal development appears to be involved in setting a long-lasting, androgen-independent transcriptional program in these cells. Our findings have broad implications in understanding the establishment and stability of sexually dimorphic traits. NCONTROLLED hemorrhage, and by way of consequence, massive transfusion MT ; is a frequent complication of trauma and surgery. MT is commonly defined as the replacement of one blood mass in a period of 24 hr. A dynamic definition of MT, such as the transfusion of four or more red cell concentrates within one hour when ongoing need is foreseeable, 1 or the replacement of 50% of the total blood volume within three hours, is more relevant in the acute clinical setting. Massively transfused patients will show evidence of defective hemostasis in a high percentage of cases. In this review, the term "coagulopathy" is used interchangeably with the more general term "defective hemostasis" and encompasses both defects of primary hemostasis related to platelet function ; and coagulopathy related to alterations of the plasma phase of coagulation ; . About 10 g of anhydrous sodium sulfate was added, and the precipitate that formed was removed by filtration. The filtrate was evaporated to about 1 15th the original urine volume.
