Technological Advances in Gene Profiling The discovery of BRCA1 and BRCA2, the genes that were found to place women at significantly higher risk of developing breast and ovarian cancer, were an important medical advance. But this is only the beginning of the role that genes will play in breast cancer treatment. By analyzing the genetic components of breast tumors researchers may be able to glean information about the tumor that can help women and their physicians make treatment decisions. Advances in this area of research were discussed in one of Symposium's general sessions and presented as posters. One of the more interesting presentations looked at using gene expression profiling to try to identify which cancers are most likely to recur in women with node-negative breast cancer. Using techniques known as microarray analysis and hybridization, the researchers compared gene expression patterns in women who had their cancers recur and women who did not. After looking at more than 100 genes they identified 20 that were about 84% accurate in predicting whether the outcome would be good or bad. Next, the researchers will continue to test these genes to see if they can replicate their results and determine which genes are the best markers. Another group of researchers looked for specific genetic defects in ductal carcinoma in situ DCIS ; , which is non-invasive, and then looked to see if these same genetic defects were seen in women with invasive breast cancer. The researchers found 114 genes that appeared in both the non-invasive DCIS and the invasive tumors. Now these researchers are going to try to determine a genetic evolutionary relationship between DCIS and invasive breast cancer. If so, this information could help clinicians fine tune treatment options for women with DCIS. This research on genes and gene expression patterns may one day help us to find breast cancers very early, especially if we can find these markers in the breast fluid that can be obtained through ductal lavage.

We examined the effect of recombinant human IGF binding protein 3 rhIGFBP-3 ; , an antagonist of IGF-IR signaling, in Herceptin-resistant breast cells in vitro and in tumors in vivo. Consistent with results obtained using HER-2- or IGF-IR-transfected cells MCF-7 HER2-18 and SKBR3 IGF-IR, respectively ; , we found that rhIGFBP-3 significantly reduced IGF-I-induced IGF-IR phosphorylation and displayed a synergistic interaction with Herceptin against cultured HER-2-overexpressing breast cancer cells in vitro. We show, for the first time, the antitumor activity of rhIGFBP-3 against advanced-stage MCF-7 HER2-18transfected human breast cancer xenografts and its potentiation of Herceptin activity. Herceptin, or Trastuzumab, is one of the first examples of a targeted, or biological therapy for breast cancer. Herceptin was designed specifically to attack the HER-2 gene that is known to be a problem for 20 to 30% of breast cancer patients. What we know now is that this new antibody against the HER-2 gene is effective not only for patients with advanced breast cancer, but for patients with early breast cancer as well. Which is a wonderful advance in preventing the disease from coming back. Exactly, and Herceptin is able to prevent breast cancers from recurring after diagnosis in nearly 50% of the patients who take it. Can you share with us some details about the new clinical trial that just started here at Yale using Herceptin? We know that Herceptin is a wonderful advance for women with a certain type of breast cancer that has HER-2 gene expression, but the exact type of chemotherapy to use with the Herceptin and the frequency is not completely clear. Our goal is to not only improve our ability to treat the breast cancer, but to reduce side effects. We have a treatment strategy that is less likely to cause hair loss and nausea. Roche and Genentech are pursuing a comprehensive clinical programme investigating the use of Avastin with a number of chemotherapeutic agents in advanced colorectal cancer and as adjuvant therapy following surgery. As Avastin's mechanism of action may be relevant in a number of malignant tumours, we are also investigating the drug's potential clinical benefit in other cancers, including nonsmall cell lung cancer, pancreatic cancer, renal cell carcinoma and breast cancer. Approximately 15, 000 patients are expected to be enrolled in clinical trials worldwide over the next several years. As Tarceva is designed to interfere with a molecular signal that stimulates tumour cell growth in numerous types of cancer, it is currently being investigated in a variety of malignant diseases by a global alliance of Roche, Genentech, OSI Pharmaceuticals and Chugai. Tarceva is also being evaluated in combination with Avastin. Major programmes exploring the role of Herceptin in adjuvant breast cancer and of Xeloda in the adjuvant setting in colon and breast cancer are continuing, as are more exploratory trials of the potential benefits of several early-stage molecules with distinct mechanisms of action in a number of cancers. In 2004 Roche scientists identified a class of small molecules that activate a key tumour suppressor pathway that protects cells from becoming malignant. To him Buddhism was a caste-cum-sect within the Hindu fold. ; Satyapriya and I then spoke for about half an hour each. Swami Agamananda concluded by appealing to the public of Muvattupuzha to support the Ashram and co-operate with its new incumbents. On our return to the Ashram a committee was set up with Swami Agamananda as President, Anagarika Satyapriya as Secretary, Anagarika Dharmapriya as Assistant Secretary, and Mr Ramaswami Iyer who, with about a dozen other gentlemen, had walked back with us as Treasurer. Ramaswami promised ten rupees a month. The others pledged one rupee each. A list of subscribers, potential and actual, was drawn up, and Satyapriya and I were instructed to go from house to house for subscriptions at the beginning of every month. From the first of December, therefore, our daily routine underwent an important modification. Every morning after breakfast we crossed the bridge into Muvattupuzha and made the rounds of as many houses as possible, returning to the Ashram at about midday. In the beginning we had assumed it would be possible for us to collect the subscriptions of all the members of the Ashram, as they were styled, within two or three days. But as our list grew longer, and the town apparently more extensive, the work lingered on into the first, into the second, and finally even into the third week of each month. Members were not always at home when we called, and even if they were they sometimes asked us to call again later as they happened to be without money just then. Moreover, with most of the subscribers on our list, who at the peak of our activity numbered nearly a hundred, we were soon on terms of friendship, even, in a few cases, of familiarity. For this development Satyapriyas extreme sociability was mainly responsible. In consequence we acquired the habit of visiting certain houses more often, and staying much longer, than our business required. SUGGESTIONS FOR THE USE OF PANCREATIC ENZYMES IN CYSTIC FIBROSIS 12 1. Patients should be receiving optimal diet for age and clinical status, recognizing that those with failure to thrive or malnutrition require additional calories and other nutrients for catch-up growth. 2. Nutrition assessment should be a part of routine clinical evaluations. 3. Initial dosing of pancreatic enzyme supplements should begin with 500 lipase U kg meal using enteric-coated microsphere products. 4. Patients should be reassessed 2-4 weeks after initiation of therapy. The following items should be assessed: Clinical status, e.g., abdominal symptoms and exam; Nutritional intake and growth height, weight, head circumference Character of stools - greasy, oily for information, not for decision making Quantitative 72-hour fecal fat when indicated but not less than annually perform on a normal diet for age Fat soluble vitamin measures. 5. Corollaries to dosing suggestions: a. Dose may be altered in a stepwise fashion according to the response of the patient see 4. above ; . b. Dose approaching 2, 000 lipase U kg meal would indicate the need for further investigation see below ; . Patients presently on higher doses should be reevaluated; either immediately decrease the dose or titrate down to a lower dose range at, or below, 2, 000 lipase U kg meal. Doses 6, 000 lipase U kg meal have been associated with colonic strictures. c. Pancreatic supplements mixed with applesauce or other acidic food substances should be administered immediately, not stored. d. Enteric-coated microspheres should not be crushed. e. Enzyme doses as lipase U kg meal ; tend to decrease with advancing age. f. Patients should accept only product brands prescribed by their physician. g. Adjustment of dosage is the responsibility of the physician. Patients should be advised not to adjust doses without consulting their physician. Changes in product or dosage may require an adjustment period. h. Complaints transmitted by phone should be investigated thoroughly before dose is adjusted. If indicated, this investigation should include 72-hour fecal fat testing. i. Pancreatic supplements should be stored in a cool, dry place and checked regularly for expiration date.
We hypothesize that lack of access to health care and to newer modalities for treatment of stage IV breast cancer, such as targeted therapies like Herceptin and aromatase inhibitors, are two contributing factors for the growing disparity, " said Dr. Giordano. Cathy Eng, M.D., Gastrointestinal Medical Oncology, and coworkers examined the impact of cetuximab, an EGFR inhibitor, on survival and health-related quality of life in patients with pretreated EGFR-expressing metastatic colorectal cancer. In patients receiving cetuximab plus irinotecan compared with irinotecan alone, they found significant improvements not only in progression-free survival and response rate, but also in global health status scores and symptoms such as pain, nausea, and insomnia. For patients who are in remission with high-risk acute myelogenous leukemia but who are elderly or have comorbidities that contraindicate myeloablative transplantation, reduced intensity allogeneic hematopoietic stem cell transplantation RISCT ; proved to produce durable long-term remission. uday Popat, M.D., and coworkers in Stem Cell Transplantation and Cellular Therapy found a three-year overall survival rate of 75%. Ehab Atallah, M.D., a fellow in the Department of Leukemia, reported that dasatinib, a second-line drug for treating chronic myelogenous leukemia, yielded high response rates when given to newly diagnosed patients as first-line therapy. The phase II trial demonstrated a complete cytogenetic response in 95% of patients at one year. Future clinical trials comparing dasatinib with imatinib, the current approved front-line therapy for CML, will be needed to determine whether treating with dasatinib first will produce better overall survival. Dasatinib looks particularly promising because it blocks both forms of BCR-ABL while imatinib blocks only one. Anas Younes, M.D., and coworkers in Lymphoma Myeloma reported preliminary results from a phase II trial of the HDAC inhibitor MGCD0103 demonstrating that this agent has significant anti-tumor activity against relapsed or refractory Hodgkin's lymphoma and is well-tolerated.
